What to bring
A photo ID, your insurance card, a list of current medications, and any recent X-rays if you’re switching from another dentist. New patient forms are available at check-in — or download them ahead of time (demo placeholder).
Insurance basics
We work with most major PPO plans and will verify your benefits before treatment so you know what’s covered.
Payment expectations
You’ll get a written estimate before any treatment. We accept cards, HSA/FSA, and offer financing for larger treatment plans.
Nervous about the visit?
